Milton Park Partners HR Forms

Most forms will require you to complete the form and then you will receive an email with a new link to confirm and sign electronically.  If you do not sign the form, then Milton Park Partners (MPP) will not be notified.


Employee action form

Used to process transactions such as status change, pay adjustments, commission, bonus, allowances for an employee.

Receipt of Company Equipment

Used for any employee that receives equipment and the same form can be used for returning equipment.

Netchex Manager Access Request

Used for viewing individuals on your team, time and attendance, PTO, payroll, and human resources.

Employee Evaluation FormS

Employee’s performance review to document highlights of an individual’s strengths and weaknesses when it comes to workplace performance.

Separation Employee Action Form

Contains form fields that ask for details about the employee like his/her name, position, and specific reason for termination.

PERSONAL ADDRESS CHANGE REQUEST FORM

Must be signed by employee to confirm change.

Employee Incident Report Form

Used to provide a comprehensive record of any unwelcome or undesirable occurrence that occurs within the workplace environment.

New Hire Employee Information

Minimum required information in order for Milton Park Partners to be able to give access to the portal .

Health savings account (HSA) Contribution Change Form

Used for making a contribution change or termination of an existing HSA allocation.
